This form is provided for California residents to exercise their privacy rights under the California Consumer Privacy Act (CCPA) and California Privacy Rights Act (CPRA).

Your California Privacy Rights

As a California resident, you have the following rights:

  • Right to Know / Access: Request disclosure of the personal information we have collected about you over the past 12 months, including categories, sources, purposes, and third parties it was shared with.
  • Right to Delete: Request that we delete personal information we have collected about you, subject to certain exceptions.
  • Right to Correct: Request that we correct inaccurate personal information we maintain about you.
  • Right to Opt Out of Sale/Sharing: Opt out of the sale or sharing of your personal information for targeted advertising.
  • Right to Limit Use of Sensitive Personal Information: Request that we limit the use and disclosure of your sensitive personal information.
  • Right to Non-Discrimination: You will not receive discriminatory treatment for exercising any of these rights.

Required Information for Your Request

To process your request, we will need:

  • Your full name
  • Email address associated with your account (if applicable)
  • Mailing address (for verification purposes)
  • Type of request (Know/Access, Delete, Correct, Opt-Out, Limit Use)
  • Specific details about your request

Verification Process

To protect your privacy and security, we must verify your identity before processing your request.

We may ask you to:

  • Confirm your email address by clicking a verification link
  • Provide additional identifying information that we can match to our records
  • For sensitive requests, we may require a signed declaration under penalty of perjury

Response Timeframe

We will confirm receipt of your request within 10 business days.

We will respond to your request within 45 calendar days.

If we need additional time (up to an additional 45 days), we will notify you of the extension and the reason for the delay.

Authorized Agents

You may designate an authorized agent to submit requests on your behalf.

To do so, the agent must provide written authorization signed by you, and we may require you to verify your identity directly with us.
